If you do not have insurance or we do not currently accept your plan please start here. Out of Network Clients can request a “superbill” to submit to their insurance and typically receive 50-80% reimbursement.
New Client/Initial Evaluation
During your initial evaluation, we will obtain a comprehensive review of your symptoms, concerns, and medical history. We will work with you to develop a treatment plan that fits with your personal values and treatment goals.

Follow Up Evaluation
Follow up visits focus on symptom management, medication tolerance, and assessing progress towards your individual treatment goals. Follow up visits are typically scheduled every 4 weeks but will vary based on individual client needs.
$200 | 30 mins
Urgent Same Day Evaluation
We offer URGENT same-day appointments for clients who do not feel they can wait for an available appointment time. This can be either an initial evaluation or a follow up evaluation and is subject to provider availability.

Base rate + $75
If you are having a medical or psychiatric emergency, including thoughts of harming yourself, others, or suicide, please call 911 immediately or go to the nearest emergency room.
Document Preparation
Disability certification, treatment verification letters, court documents, emotional support animals, FMLA leave, school accommodation letters, etc. Document fee will be charged upon completion of the form.
